## Description
**GENERAL DESCRIPTION OF POSITION:**Educates families and individuals in Cameron and Willacy Counties about Su Clinica Services and of health services availability, accessibility and eligibility by providing educational outreach services. Promotes a healthier community by providing preventative health education. Provides voter registration as a Certified Deputy Voter Registrar for Cameron/Willacy Counties. Participates in Community Health Center Advocacy Campaigns advocating for access to healthcare. Provide referrals and patient registration follow-up increasing the number of patients we serve. Able to promote Su Clinica and outreach activities through social media posts, engagement and monitoring professional content on approved social media platforms, adhering to all Su Clinica social media policies. Works closely with the Outreach Team to reach goals and objectives of program and to follow and support the vision and mission of Su Clinica at all times. Through community partnerships participates and provides community events/presentations to promote healthy living and preventative health education.
**ESSENTIAL JOB FUNCTIONS:** (with or without accommodations)
**Patient Relationship:** Will greet and welcome the patients/clients in a pleasant, respectful, culturally and linguistically appropriate manner. Assure all patient/client questions are answered in a positive and cordial manner. Assures the patient/client has a clear understanding of all communications when providing preventative health education and/or basic clinic information. Provide hospital visits to new mothers, home visits as needed and provide support as needed to patients at the clinic who are on the WH Caseload given to each Outreach Worker.
**Manages patient/client information:** Adheres to all HIPAA regulations in and outside of the Clinic. Communicates patient information as requested to enhance patient services. Provide information on Su Clinica as a medical home and obtain/track new referrals through first appointment status. Provides assistance in the registration process and offers tours and clinic service information to those interested in services. Reinforces to the patients the importance of keeping appointments or communicating changes in their appointments. Schedules appointments and provides support services as needed to each clinic site.
**Community Supports:** Coordinates and/or participates in community health fairs/presentations to promote a healthy lifestyle. Distributes informational printed materials such as posters, flyers, brochures & other promotional items. Develops collaborative community relationships with local institutions such as employers, faith based organizations, schools & other community based social service agencies to promote the goals of our program. Conducts lobby presentations within Su Clinica to promote healthy living and to identify patients who could benefit from other services such as Women’s Health, Pediatrics or Pedi Dental. Will focus on increasing new patients to pediatric clinic, pedi dental & Women’s Health. Provides Breast Feeding Peer Counseling and works with pregnant patients of Su Clinica to provide health information promoting healthy outcomes for child birth. Meets with new mothers at the hospital to provide assistance in scheduling first appointment for newborn. Coordinates Su Clincia sponsored events to promote health, wellness & clinic services. Provides Voter Registration & maintains Deputy Voter Registrar status. Promotes Community Health Center Advocacy as directed. Acts as liaison between health center, clients & community resources. Participate on community coalitions to enhance goals of program. Develops collaborative community relationships with local institutions such as employers, faith based organizations, schools and other community based social service agencies to promote the goals of the program.
**Demonstrates Outreach Skills and Abilities:** Coordinates special events and fundraising activities for Su Clinica. Supports Community Health Center Advocacy Campaigns. Will refer clients to Outreach CAC’s for application assistance on Marketplace, CHIP and Medicaid. Agrees to participation with outreach efforts to engage young children using a variety of methods to include games, activities and costumes/mascots. This may include wearing costumes/mascots to engage children and provide a positive fun experience to children/families during special events. Prepares monthly reports on outcome measures of activities, number of families encountered, number of new unique users who received first appointment status. Reports will be detailed, accurate and timely. Reports will be based on Su Clinica Strategic Plan, Su Clinica Business Plan, Administrative Directives and the goals and objectives of the Outreach Department. Will be responsible for your own safety and the safety of others. Will obtain and maintain Breastfeeding Peer Counselor status. Must demonstrate verifiable social media skills, including the ability to create content, post, monitor, and maintain professional content on approved social media platforms.
**Team Communication:**
Actively participates in team huddles and staff meetings to assure focus is on goals and objectives. Meets with the team to ensure that the productivity and schedule is reviewed and that all activities are covered. Helps in the orientation of new employees or volunteers. Works with Outreach team members as well as other clinic personnel to ensure a smooth and successful visit for new patients referred by Outreach. Communicates special event information to other departments within the clinic that may be effected and/or need to know. Provides timely and accurate reports as required.
**Demonstrates Safe Professional Conduct:**
Maintains a friendly and safe environment for self and others. Refrains from texting and speaking on personal cell phone while on duty. Will maintain the phone on mute to avoid hearing the ringing of personal cell phone. Fully participates in performance improvement and follows all clinic policies and procedures. Responsible for own safety and the safety of others. Attends work on a regular predictable schedule in accordance with clinic leave policy notifying Director of Outreach for all absences. Performs other duties as assigned. Submits required documentation in a timely manner (credentialing requirements, license renewals, certifications, continuing education attendance, etc.) Will fully comply with the enhanced infection control requirements of the clinic at all times. Approaches patient care as a team involving clinical and non-clinical staff (eg. Physicians, mid-levels, educators, appointment staff) to achieve patient care objectives.

## Qualifications
**KNOWLEDGE, SKILLS, AND ABILITIES:** (use of equipment, job related knowledge, language, etc.)
Ability to fully comply with the enhanced infection control requirements of the clinic
Knowledge and skilled in use of computer and computer software (EMR, Microsoft Word, Excel, etc.) programs as necessary
Knowledge of community resources
Knowledge of SC policies and procedures.
Knowledge of the Chronic Care Model and Patient-Centered Medical Home Model (PCMH).
Knowledge of community demographics
Knowledge of outreach techniques
Knowledge of social media skills with ability to demonstrate content creation
Skilled in the preparation in the conducting of group presentations
Skilled in coordinating activities
Skilled in excellent customer service
Obtain and Maintain Breast Feeding Peer Counselor Certification
Obtain and Maintain Texas Deputy Voter Registrar Certification
Ability to schedule patients as needed for appointments
Ability to prepare records/reports in accordance to detailed instructions and complete projects independently
Ability to work with minimal supervision

Ability to complete Compliatric Requirements in timely manner.
**MINIMUM QUALIFICATIONS/CERTIFICATES/LICENSES/REGISTRATIONS REQUIRED:** Bachelor’s degree in human services or health related field plus one year of experience or associate’s degree plus 3 years of experience in public-facing/customer services and information gathering.. Public speaking experience preferred. Must demonstrate verifiable social media skills, including the ability to create, post, monitor and maintain professional content on approved social media platforms. Driver’s license and dependable transportation required. Bilingual English/Spanish.
**SPECIAL INSTRUCTIONS:** This position requires travel. Employee must provide transportation. If employee operates a personal motor vehicle in the performance of their official duties, the employee must possess a current valid Texas driver’s license for the appropriate type of vehicle and Texas Liability insurance. This position requires that an employee’s driving record be verified with law enforcement to ensure compliance with SCF’s driving policy.
Due to the nature of Outreach Services it will be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community. You will be expected to work on Saturdays occasionally as well as flex hours during the week to maintain a 40 hour work week.
