## Description
Job Summary: In accordance with the organization’s mission, vision and values, the LPN OTP is responsible for the daily dispensing of Methadone. The following duties will apply:
**Duties**

  1. Coordinate and document medical program compliance in accordance with Agency & OASAS policies, procedures and regulations.
  2. Dispense Methadone medications accurately to OTP clients.
  3. Accurately administer injectable medications to outpatient clients as needed and follow up with proper documentation.
  4. Monitor patient inductions by obtaining precise vital signs and thoroughly documenting responses to Methadone.
  5. Execute medical orders by facilitating medication refill requests and processing insurance prior authorizations.
  6. Perform clinical tasks including administering EKGs and collecting urine drug screens.
  7. Ensure that all documentation for Methadone dispensing is recorded accurately.
  8. Completes Medication Administration Records and Chain of Custody forms for residential patients receiving Methadone.
  9. Maintains up to date inventory on all supplies for OTP program.
  10. Maintain flawless records by accurately completing Medication Administration Records (MARs) and Chain of Custody forms.
  11. Print and archive daily Methadone administration documentation according to legal retention policies.
  12. Initiate COTI referrals promptly for clients missing doses for 2 or more days.
  13. Orients new clients to the Methadone program and answers client questions.
  14. Monitors and documents Methadone supply twice daily.
  15. Maintains electronic and manual Methadone pumps and reports any concerns.
  16. Participate in general, staff, in-service and supervisory meetings held by the Agency.
  17. Participates in agency/program continuous improvement/department enhancements related to medical/nursing.
  18. Provides DON/OTP and counselors with verbal communication updates of client status or more frequently as necessary.
  19. Responds appropriately to crisis and emergency situations in order to ensure/secure the health and safety of the clients. Adheres to regulatory guidelines and follows-up by appropriate communication and documentation to identified personnel.
  20. Assures that all clients’ required record documentation is accurate and timely and in compliance with Agency and regulatory policies, practices and procedures.
  21. Perform other duties and functions as directed.

## Qualifications

Minimum Qualifications: Valid NYS LPN. Valid NYS Drivers’ License. Behavioral Health and Substance Use Disorder background preferred. CPR Certification will be required within thirty days of hire.

**Other Skills/Knowledge and Experience**

  1. Excellent written and verbal communication skills.
  2. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.
  3. Working knowledge of behavioral health.
  4. Ability to multi-task, have good problem solving & time management skills.
  5. Recognition and understanding of the scope of practice according to the Nurse Practice Act.
