About The Role

## Description

Job Purpose

  • The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER is responsible for providing high-quality nursing care to patients within the facility. This role requires a dedicated and compassionate individual who can work in a pool capacity, offering flexible support to various departments as needed. The LPN will utilize their extensive experience to ensure the delivery of effective and efficient patient care while adhering to established protocols and standards.

Key Responsibilities

  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient health by assessing vital signs and reporting any changes in condition to the healthcare team.
  • Assist in the development and implementation of patient care plans.
  • Provide direct care and support to patients, including assistance with daily activities and personal hygiene.
  • Maintain accurate and up-to-date patient records and documentation.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Educate patients and their families on health management and preventive measures.
  • Ensure compliance with all healthcare regulations and facility policies.
  • Participate in ongoing training and professional development opportunities to enhance nursing skills and knowledge.
  • Support a safe and therapeutic environment for patients and staff.

## Qualifications

Required Education

  • Completion of a state-approved Licensed Practical Nurse (LPN) program.
  • Current LPN license in the state of practice.

Required Experience

  • Minimum of 6 years of experience working as a Licensed Practical Nurse, preferably in a behavioral health or similar healthcare setting.
  • Demonstrated experience in providing nursing care and administering medications to patients with mental health conditions.
  • Experience in collaborating with multidisciplinary teams to develop and implement patient care plans.

Required Skills and Abilities

  • Strong understanding of behavioral health principles and the ability to apply them in a clinical setting.
  • Excellent communication skills, both verbal and written, for effective interaction with patients, families, and healthcare team members.
  • Ability to maintain composure and professionalism in high-stress situations.
  • Proficient in the use of electronic health records (EHR) and other healthcare-related software.
  • Strong organizational skills with the ability to prioritize tasks and manage time efficiently.
  • Compassionate and empathetic approach to patient care, with a commitment to enhancing patient well-being.
