Physical Therapist - PRN - Outpatient Therapy (2548)

## Description

• Evaluates each individual patient’s strength, motion, function, sensorium, balance and coordination, etc. in terms of diagnosis, precautions, and complications
• Develops, documents and implements appropriate treatment plan to promote the optimal outcomes with regard to transfer skills and mobility skills
• Maintains current and accurate records of evaluation findings, treatment programs, progress and treatment units
• Responsible for identifying and organizing solutions to difficulties in patient assessment, planning and treatment
• Interacts with the interdisciplinary team to achieve effective inter-team communication regarding patient treatment, goals, progress and discharge recommendations
• Maintain accurate and up-to-date patient records

• Clinton Outpatient Therapy
• Gluckstadt Outpatient Therapy
• AT Outpatient Therapy

**Shift:**

• PRN - As Needed
• Monday - Friday

## Qualifications

• Degree in Physical Therapy from an accredited program
• Must have current MS license to practice physical therapy
