## **Position Overview**
The Community Outreach Partner supports the execution of localized, territory-based community engagement and referral development for Flournoy Health Systems across Georgia. This role is responsible for building strong local relationships, developing referral sources, increasing community awareness, and contributing to a consistent, measurable referral pipeline across assigned regions.
The Community Outreach Partner serves as a key representative of Flournoy Health Systems within the communities it serves. This role focuses on territory-specific execution through direct relationship-building with local organizations, referral sources, healthcare partners, and community stakeholders. The Community Outreach Partner is accountable for prospecting, outreach activity, referral source development, partnership follow-up, territory visibility, and accurate CRM documentation to support a scalable, high-performing referral pipeline across all assigned territories.
## **Key Responsibilities**
- Execute territory-based community outreach plans aligned with organizational growth and referral goals
- Build and maintain strong relationships with referral sources, community organizations, healthcare partners, and local stakeholders across assigned territories
- Identify, prospect, and develop new referral source opportunities to strengthen the local pipeline and expand territory reach
- Maintain regular in-person outreach activity across the territory to ensure consistent visibility, relationship development, and local market presence
- Represent Flournoy Health Systems professionally in the community through meetings, events, networking opportunities, and partnership development efforts
- Support referral growth across all service lines, including Structured Family Caregiving, traditional caregiving, Pediatrics (skilled nursing and personal support), and Hospice care (and new services)
- Track all outreach activity, referral source engagement, follow-up tasks, and partnership updates within the CRM in real time
- Operate fully within the CRM, maintaining accurate data, timely follow-up, clear account ownership, and visibility into territory activity and pipeline development
- Follow established outreach workflows, processes, operating rhythms, and accountability standards to ensure consistency across territories
- Partner closely with leadership to communicate market feedback, local opportunities, relationship risks, and territory needs
- Monitor individual outreach performance, referral activity, and account development progress against defined goals
- Support territory planning and outreach initiatives tailored to local market needs and community dynamics
- Maintain a high level of responsiveness, organization, professionalism, and follow-through in all relationship and referral development efforts
- Collaborate with peers across different regions and territories to support organizational growth and relationship transition when required
- Travel regularly throughout assigned territories across Georgia to support community presence and outreach execution
## Qualifications
## **Qualifications**
- Bachelor’s degree or higher preferred
- 3+ years of experience in healthcare-related sales, community outreach, relationship management, business development, field marketing, or related fields
- Fluency maintaining sales data in CRM. Experience with Hubspot is a plus.
- Healthcare experience required; home care, home health, hospice, or Medicaid-funded programs strongly preferred
- Proven ability to build relationships, develop local partnerships, and support referral growth within assigned markets
- Strong communication, presentation, and interpersonal skills
- Professional, polished presence with the ability to engage a wide range of community and healthcare stakeholders
- Highly organized, self-directed, and comfortable managing outreach activity across a defined territory
- Results-driven with strong follow-up, accountability, and work ethics
- Working knowledge of Medicaid and Medicare programs is a plus
- Valid driver’s license, reliable vehicle and ability to travel regularly across Georgia
