Family Tree Rehabilitation, part of **The Campus of Anna Maria**, is seeking a **Physical Therapist (PT)** to support our therapy team as needed. Situated on a beautiful **wooded campus** in Ohio, we are a **family-owned and operated skilled nursing facility** dedicated to providing high-quality, resident-centered rehabilitation services in a collaborative and supportive environment.

**Position Summary**

The PRN Physical Therapist is responsible for evaluating residents, developing and implementing individualized physical therapy plans of care, and helping residents improve mobility, strength, balance, and overall functional independence.

**Key Responsibilities of Physical Therapist (PT)**

  • Perform physical therapy evaluations and establish individualized plans of care
  • Provide skilled physical therapy treatment services in accordance with physician orders
  • Collaborate with and supervise PTAs as applicable
  • Accurately document evaluations, treatments, progress notes, and discharge planning
  • Participate in interdisciplinary care planning and team communication
  • Ensure compliance with facility policies, professional standards, and regulatory requirements

## Qualifications
**Qualifications for Physical Therapist (PT)**

  • Current Ohio Physical Therapist (PT) license in good standing
  • Graduate of an accredited Physical Therapy program
  • Experience in skilled nursing or rehabilitation setting preferred
  • Strong clinical judgment, communication, and time-management skills
  • Compassionate, resident-centered approach to care
