Nurse Practitioner

Reporting to the Director, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens) as determined by community and program needs. Depending on community and team requirements, the position may include first on-call support. As the PHC Team evolves and based on needs of the population, the position may be required to provide new practices such as nurse led extended hour clinic services and/or first on-call support.
