About The Role

Evaluates medication orders for contraindications; drug-drug interactions; drug-food interactions; allergies and appropriateness of drug, dose, frequency and route. Formulate patients’ treatment plans and initiate, modify, and continue appropriate drug therapy in accordance with HIV standards of practice or in consultation with the primary physicians. Initiate requests for appropriate laboratory and other diagnostic studies as needed for monitoring drug therapy. Provide treatment adherence counseling and support for patients in the HIV Treatment Adherence Program. Assists patient and multidisciplinary team with solving medication issues (i.e., prior authorization, patient assistance programs, and financial support programs). Act as a preceptor for pharmacy students and residents in clinic. Provides educational information for patients and families, program staff, and other patient care professionals. This may be in the form of a continuing education presentation or other department-wide communication. Knowledge, Skills Abilities: Strong understanding of HIV pharmacy services. Ability to travel to different clinic sites. Possess a high level of organizational skills with the ability to multi-task, and track both long- and short-term projects while performing day-to-day tasks. Strong communication skills with the ability to effectively work with a multidisciplinary team to solve problems or improve operations. Demonstrate personal responsibility, respect for self and others, innovation through teamwork.
