About The Role

Your future role at a glance Location: Bartlesville, OK Facility: Ascension Jane Phillips Medical Center Department: Total Quality Management Schedule: Full Time, Monday-Friday, 11am-7pm How you'll make an impact in this role Serve as a critical bridge between the hospital and the community. Facilitate resource navigation for patients during their emergency department visit. Refer complex clinical needs to Behavioral Health or RN Care Managers. What minimum qualifications you'll need Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required What additional requirements you'll need Strong understanding of community and public resources available to patients who are on Medicaid or uninsured. Must be able to engage patients, build trust quickly, and assess their level of motivation and need. Demonstrate sensitivity to diverse populations and an understanding of unique cultural barriers to care. Excellent communication skills and the ability to refer patients to appropriate Case Managers for continued care.
