Intake Specialist - Client Advocate

As an Intake Specialist, you'll meet with Persons Served and their families in their homes to evaluate and assess individual needs and develop personalized support plans. Schedule and coordinate timely follow-up visits to ensure quality service delivery and continuity of care by Direct Support Professionals. Bring and complete all necessary paperwork to the home visit with the Person Served.
