Registered Nurse (RN) Care Manager (RNCM) Overview: Aptive Resources, in partnership with Valor Healthcare, is hiring a Registered Nurse Care Manager to provide care coordination and chronic disease management within the PACT model at the proposed Yreka, CA CBOC, pending VA contract award. Location: On-Site | Yreka VA Community-Based Outpatient Clinic (CBOC), 807 S. Main St., Yreka, CA 96097 Schedule: Monday–Friday, 7:30am–5:30pm (Clinic Operating Hours); 8:00am–4:30pm Patient Schedule Hours
