Director of Case Management

Bachelor's Degree required. Master's Degree preferred. Minimum of seven (7) years related experience in a health care setting, with at least five (5) years in a management or leadership capacity. Current knowledge of trends and techniques in patient care. Demonstrates understanding of assessment, planning, and managing patient care. Familiarity with all state and regulatory requirements related to case management and social work. Current NYS Licensure as a Registered Professional Nurse or Social Worker required. Certification in Case Management, PRI Certification preferred.
