Rapid Response Nurse Ottawa (Full-Time Permanent)

Reporting to the Manager of Transition Programs, the Rapid Response Nurse (RPN), is responsible for the provision and management of quality nursing care to clients transitioning out of hospital as part of the Carefor’s atHome bundled care programs. The Rapid Response Nurse (RPN) is responsible for managing the wrap-around care that is coordinated and delivered to clients admitted to the program, with support from members of the care team.
