Case Manager- La Crosse/Sparta

We are seeking a Case Manager to provide comprehensive case management services to frail elders and adults with intellectual, developmental, or physical disabilities within the Wisconsin Family Care programme. Key responsibilities include developing Member Care Plans and coordinating with field care teams. Ideal candidates will have experience serving vulnerable populations and a background in human services.
