About The Role
The Clinical Pharmacist monitors the medication usage for participants to ensure that it is safe, effective, cost efficient and contributes to optimal participant outcomes. In addition, the candidate will be a drug information resource for the medical and nursing teams, participants, and their families. Medication Therapy Management - Evaluates medication regimen of participants upon intake and on a periodic basis based on patient’s conditions, and on a semiannual basis and makes the following clinical interventions: -
