RN Community Wellness-Community Outreach

The Patient Educator position duties include, but are not limited to the following:- Assist the Community Outreach Department with organizing, implementing and maintaining wellness activities by utilizing their professional knowledge and experience as it relates to their field of expertise- Regularly participate in and promote wellness programs, including public presentations, with concentration on health promotion, disease prevention, screenings and lifestyle modification- The Patient Educator will refer clients as needed to KHN physicians and services and will track and measure outcomes as mandated- Will act as the liaison at events and will promote and use service lines in the development of programs specifically designed to meet the needs of the community- The Patient...
