Dietitian

The Dietitian is responsible for providing safe, evidence-based, and patient-centered nutrition care for both adult and pediatric populations in accordance with Department of Health (DOH) regulations and Americare Home Health Services’ policies and procedures.

The role involves conducting nutritional assessments, planning and implementing individualized dietetic interventions, and monitoring outcomes for patients with a range of medical conditions across post-acute rehabilitation, long-term care, homecare, and outpatient settings.

The Dietitian is expected to maintain and update professional knowledge and clinical competencies to ensure high-quality care that aligns with current best practices and accreditation standards (DOH, JCI, and CARF).
