Care Navigator

The Care Navigator will identify barriers to care, assist with access issues, develop relationships with health care providers, and ensure effective communications with providers, patients, and caregivers to coordinate services within the organization, at outside facilities, and within the community. The Navigator will track admissions/readmissions, and how patients are helped, and the results of the help given. The Care Navigator will also track and help with Bundle Payments and comprehensive Joint replacement. The Navigator will partner with stake holders to raise awareness about hospital/support programs and services, community resources and needs articulated by patients and families.
