Nurse Practitioner

The Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers. Responsibilities : Involve with direct patient physical assessment, education, and health planning Maintain continuity of care by regular patient follow-up Assist in the coordination of services with other disciplines and patients' insurance plans Advocate for patient in arrangement of medically necessary services Communicate with community resources to ascertain adequate assistance for patients in need Refer patients to specialty services when necessary Consult with collaborating physician and other members of the health care team to establish plan of care Participate in quality assurance process to assure optimal care delivery Provide comprehensive case management services that include emphasis on health promotion and disease prevention Monitor patient outcome in accordance to the practice protocols Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning Act as a resource person for the nursing staff Conduct education in service on practice issues Perform other duties as assigned
