About The Role

As a member of a multi-disciplinary treatment team provide direct care as delineated in the CTP
Provide therapy as clinically indicated for individuals and family units
Engage all consumers and their family members in treatment planning and all areas of service delivery
Assist consumers with ADL's through modeling, direct assistance and coaching
Provide assessment of client's emotional well-being, clinical symptomology, behavioral changes and medication compliance.
Evaluate significance of SU history and current use if applicable
Provide psycho-education for consumers and family members on mental illness, symptom management, medication compliance and life skills
Master’s Degree in Social Work, Mental Health or Human Services Related Field
LSW/LAC or LCSW/LPC required
Experience with entitlements and community system of care
