About The Role
Key Responsibilities
- Under supervision:
- Provides supportive assistance with activities of daily living (ADLs) through modeling, direct assistance, and coaching.
- Assists clients in obtaining basic needs.
- Assess client emotional well-being, clinical symptomology, behavioral changes and medication effectiveness.
- Provides psychosocial education for consumers related to mental illness, symptom management and medication compliance.
- Provides crisis intervention as necessary.
- Provides rotating 24 hour on call coverage as scheduled.
- Maintain relationships with state and local hospitals.
- Participate in daily treatment team meetings and weekly treatment team planning meetings.
- High School Diploma or equivalent
- Bachelor's Degree preferred
- One year of experience in the provision of social services for individuals with severe persistent mental illness
- Some lived experience
