About The Role
PURPOSE
- Oversees and provides psychosocial rehabilitation services which provide skill development activities,
- educational services, and pre-vocational training and transitional and supported employment services to
- individuals with severe and persistent mental illness.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Deliver educational activities, which include assisting the client in securing needed education
- services such as adult basic education and special interest courses; and prevocational services,
- which focus on the development of positive, work habits and participating in work activities.
- Provide transitional or supported employment services to facilitate client entry into competitive
employment.
- Visits clients and collateral contacts to observe clients and services; ensures client access to
- services; advocates for client rights; ensures least restrictive environment and treatment alternatives;
- supports client/family in achieving rehabilitative goals; monitors medication compliance; provides
- emergency crisis intervention, and after hours on-call services.
- Responsible for carrying out recipient's Person-Centered Plan; conducts individual and group
- interventions and skill activities; develops skills training programs; interacts with clients and families;
- teaches independent living skills and provides encouragement and advice on methods for
- maintaining appropriate behavior; develop community support; manages at-risk behavior and
- performs crisis intervention.
- Provide client services including assessing the client need for comprehensive services, linking the
- client and/or family to needed services and supports, monitoring the provision of services and
- supports, assessing the outcomes of services and supports, convening the Treatment Team
- meetings to coordinate the provision of multiple services and ensure appropriate modification of the
- PCP over time.
- Assist in the development and implementation of any other education and support groups deemed
beneficial.
- Attend trainings on recovery, psychiatric rehabilitation, trauma, supported employment practices,
- substance abuse and cultural competency. Keep apprised of new research and developments in
- these areas through journals, web sites and literature.
- Assist individuals in developing and practicing the skills defined on their Person-Centered Plans.
- This may include accompanying individuals to appointments, leisure activities or other activities of
- interest including religious, educational, or vocational to provide support and encouragement.
- Link individuals to community resources and peer supports to promote community integration.
- Actively engage individuals in services including outreach visits at the individual's home, crisis center
- or outpatient facility. Ensure all required documentation is completed in a timely, comprehensive, and
- legible manner.
- Collaborate with involved personnel in the community such as GED instructors, college
- professionals, faith-based representatives, volunteer, and employment supervisors as assigned by
- Program Manager.
- Consult and collaborate with all team members including psychiatrist on a regular basis.
- Be aware of and follow company policies and procedures including those regarding confidentiality.
- Participate in quality improvement and data gathering as assigned.
- Be aware of financial liabilities and coordinate any financial changes with the billing department and
assist in securing their party resources when indicated.
- Assist individuals in gaining access to needed psychiatric, medical, social, residential, financial, and
other services appropriate for the individual.
- Serves as role model and mentor for both staff and consumers; demonstrates the desired behaviors,
- and coaches staff in the use of approved techniques and strategies for effective behavior
- management and modification; assists clients with crisis interventions as needed; instructs; provides
- emergency response coverage or coordination as per program guidelines.
- Develops supportive relationships with consumers while attending to his/her continuity of care,
- treatment and service responsibilities as directed by Chief Operating Officer, Clinician and/or
- Licensed Professionals ensuring that the needs of the clients are met.
- Participates in quality assurance and improvement activities as set forth by the Quality Assurance
Director.
- Present in developing and implementing services.
- Performs other duties as assigned.
MINIMUM QUALIFICATIONS
- Excellent interpersonal skills; demonstrated ability to develop and maintain effective, cooperative,
and productive work relationships with supervisor, subordinates, residents, and the general public.
- Demonstrated ability to use good judgment in selecting, adapting, or applying methods or
- procedures to fit unusual or complex situations, and/or consult supervisor on new or unique
- situations; ability to adhere to policies and procedures and/or develop practical solutions in the
- absence of such.
- Ability to effectively communicate both verbally and in writing.
CONDITIONS OF EMPLOYMENT
- Satisfactory Background Investigation
- Valid Driver’s License and proof of personal vehicle insurance coverage
- Must be available to work 4 Saturday per month on a rotating schedule
- Non-Violent Crisis Intervention Training
- Ability to successfully complete service-specific training courses
PHYSICAL DEMANDS
- While performing the duties of this job, the employee is regularly required to perform physical activities,
- drive, stand and walk. In addition, the employee may be required to restrain a resident or physically
- intervene to resolve an altercation. The physical demands described must be met by an employee to
