Responsibilities
- Accept and treat patients within your designated geographic area 
- Provide 1:1 treatment
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
- Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
- Educate family members, home health aides, and other caregivers on proper therapy techniques
- Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
- Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
- Provide timely and accurate documentation for each visit
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
- Must be willing to treat geriatric patients.
 
- Occupational Therapist Program Graduate
- Licensed Occupational Therapist in the state of New York or eligible.
- Mandatory Certifications: First Aid, AED, & CPR Certifications.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
- Ability to travel to patients’ homes within the assigned geographic territory
- Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.
The anticipated base hourly range for this position is $75-90. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
