Support Worker (High Wycombe)

We're looking for a compassionate and motivated Support Worker (Community Connector) to join our Assessment & Engagement Team in High Wycombe . This is an exciting opportunity to play a key role in helping people affected by drug and alcohol use access the support they need, engage with treatment services, and begin their recovery journey.

As part of One Recovery Buckinghamshire (ORB) —a partnership between Inclusion (MPFT NHS Trust), Aquarius, and Build on Belief—you'll be joining a dedicated team committed to delivering high-quality, person-centred support across Buckinghamshire.

No two days are the same. You'll work with people from a variety of backgrounds, helping them overcome barriers, build confidence, and access the services that will support their long-term recovery and wellbeing.

About the Role

As a Support Worker (Community Connector) , you'll provide practical advice, advocacy, and person-centred support to individuals accessing our service. Working closely with colleagues across the Assessment & Engagement Team, you'll support people from their initial contact with the service, helping them navigate their recovery journey while ensuring they receive the right support at the right time.

You'll contribute to assessments, support recovery planning, and work collaboratively with health, social care, housing, and community partners to help individuals achieve positive, sustainable outcomes.

What You'll Be Doing

• Welcome and engage individuals accessing the service, helping them feel supported from their first point of contact.
• Provide advice, information, advocacy, and practical support to people affected by drug and alcohol use.
• Assist with assessments, care planning, and elements of case management under the guidance of senior practitioners.
• Build positive, trusting relationships that encourage engagement and long-term recovery.
• Support individuals to identify their strengths, goals, and aspirations, developing personalised recovery plans.
• Signpost and connect service users with health, housing, employment, education, and wider community services.
• Work collaboratively with partner agencies to ensure coordinated, holistic support.
• Maintain accurate and confidential records in line with GDPR and organisational policies.
• Promote safeguarding, wellbeing, equality, diversity, and inclusion in all aspects of your work.
• Contribute to a supportive team environment and participate in regular meetings and service development.

What We're Looking For

We're looking for someone who is empathetic, approachable, and passionate about supporting people to achieve lasting change.

You'll ideally have

• Experience working within substance misuse, social care, housing, mental health, community services, or another supportive environment.
• An understanding of the challenges associated with drug and alcohol use and a commitment to person-centred, recovery-focused support.
• Excellent communication and interpersonal skills, with the ability to build trust and motivate others.
• Good organisational skills and the ability to maintain accurate records and manage competing priorities.
• Knowledge of safeguarding, confidentiality, health and safety, and professional boundaries.
• An NVQ Level 2 or 3 in Health & Social Care (or willingness to work towards this qualification).
• A flexible, positive approach and the ability to work effectively as part of a multidisciplinary team.
• A genuine passion for helping people improve their health, wellbeing, and independence.
